Life Skills Case Manager – Valley Youth House – West Chester, PA

Chester County Independent Living Case Management program is designed to help young people, ages 16-21, currently or formally in the foster care system. The program provides assistance in developing and implementing an individual plan for independence, including housing, education, employment and life skill goals.

The Life Skills Case Manager is responsible for:

Meeting with youth consistently to help them reach their goals in housing, employment, education, and other life skills goals.
To be the primary liaison contact with children, youth and families and provide regular ongoing case coordination with community agencies, school, probation, etc.
To complete documentation in established time frames.
To respond to crisis situations in a proactive manner.
To represent the services as scheduled in court
Able to travel to sessions held in the youth’s home.
To serve as a positive role model for youth and other program/agency staff.

The ideal candidate will possess:

A commitment to positive youth development and a strengths-based approach.
Bachelor’s Degree in Social work, Human Services, or a related field.
Child Abuse, Criminal Background and FBI Clearances.
Driver’s license and vehicle (road warrior)
Able to work a flexible schedule.
Available to work evenings.
Enjoys working with adolescents.
