Community Health Organizer – Philadelphia – Pennsylvania Health Access Network

The Pennsylvania Health Access Network (PHAN) is a consumer-driven health advocacy organization with an immediate opening for a contracted community health organizer. Our passion is strengthening the voices of healthcare consumers in a statewide movement to improve the quality, value, and affordability of healthcare. Join our efforts to ensure everyone has access to high quality, affordable healthcare. PHAN works every day in rural and underserved communities, servicing over two thirds of Pennsylvania’s counties, and connecting thousands to coverage.

The community health organizer will identify, recruit and follow up with individuals and their families to empower them to make better decisions regarding their health and healthcare, as well as engage in PHAN’s grassroots advocacy campaigns. This position will directly engage individuals one on one and in group settings within the local Philadelphia community. Interested applicants should be based in Philadelphia. This is an independent contract position that will end December 31, 2016.

Responsibilities Include:
Engage individuals in discussions about their health
Identify individuals to participate in PHAN’s grassroots campaigns to improve health and lower costs
Connect uninsured individuals to PHAN’s enrollment team
Conduct community based meetings on health and healthcare
Develop community based partnerships
Work with individuals to share their personal stories with policymakers, the media and through social media
Develop grassroots leadership among individuals and community based partners

Qualifications:
Intermediate skill level of Microsoft Office products, including Word, Excel, and Outlook, and the ability to learn PHAN’s data collections system (Salesforce)
Excellent verbal communication skills (Experience working in a person-to-person and group environment is essential)
Positive and persuasive leadership skills with the ability to be a natural advocate committed to social justice
Experience working with low-income, immigrant communities and/or with diverse populations
Bilingual skills are a plus

Ability to work independently
ยท Familiarity working with populations facing multiple chronic health conditions, particularly those experiencing homelessness and individuals with Substance Use Disorders

This is a full time contract position that will end December 31, 2016
