Job Description: Transitional Living Program Coordinator

Hours: Full-Time

Job Site: Transitional Living Program

Reports To: Executive Director

Exemption Status: Exempt

POSITION SUMMARY

The Transitional Living Program Coordinator will be responsible for the day-to-day overall program operations of the Transitional Living Programs on a daily basis.

RESPONSIBILITIES

Oversee Case Management Department

Maintain supervision over Case Managers’ active caseload of residents within the program
Monitor resident files to ensure they meet grant and program expectations
Make sure Case Managers are entering data into the ETO software, are administering the Ansell Casey Life Skills Assessment to residents, are monitoring resident medications, and any other case management needs for residents
Assist in developing and reviewing appropriate service plan with Case Managers
Oversee that staff are consistent with clients following the guidelines and expectations of the program, giving consequences and following through with them
Monitor all discharged clients to ensure Case Managers are closing the files in a timely manner and are reaching out to residents after discharge to monitor their progress
Monitor all runaways in the program and make sure runaway program is in compliance with HHS and EEC

Administration

Oversees the administrative functions, policy, procedures and process are being followed
Monitor program compliance with all grants and funding requirements
Provide advocacy, crisis intervention, and supportive counseling for Staff
Develop the staff schedule
Responsible for finding coverage for open shifts
Perform monthly fire drills and log them in the fire drill log
Collects and submits monthly program stats to Program Director
Responsible for stocking house supplies i.e., toiletries, blankets, etc.
Train new employees
Oversees PULSE Volunteers, interns, and other volunteers
Arrange the purchasing of food and other items
Participate in bi-monthly program staff meeting
Participate in monthly meeting with counseling staff
Participate in monthly all staff meeting
On-Call responsibilities

Other

Other related tasks as required/requested

QUALIFICATIONS

B.A. Degree or related degree or 5-8 years experience working with adolescents, at-risk youth between 18-24, young mothers or pregnant women

Five to eight years of previous experience in residential management and administration, or recreational counseling and supervising others
Continuous clean driver’s record
Valid Licenses as required

SUPERVISORY RESPONSIBILITIES

Weekly supervision with Executive Director
