Team Leader (Licensed) – OPCC – Santa Monica, CA

Responsibilities:

Under the supervision of the Program Manager, the Licensed Team Leader is responsible for:

Street outreach to identified homeless persons working with the team to engage in services and accept treatment from the program staff.
Facilitate a team meeting to discuss the status of each client.
Manage the team’s outreach and engagement process and delivery of ongoing services.
Be the point of contact throughout the day to address client crises and emergent needs.
Complete diagnostic assessments.
Provide crisis intervention, counseling/therapy, and other interventions that promote mental health wellness and recovery, carrying a caseload of 10.
Oversee the documentation of services by the PATH team in accordance with the standards of the Department of Mental Health.
Other duties as assigned.

Qualifications:

Hold a clinical license in the State of California (LCSW, MFT, or Ph.D) and be in good standing with licensing board.
Licensed over 2 years preferred but not mandatory.
Minimum one year of experience working with chronically mentally ill adults who are homeless.
Knowledge of and expertise in providing therapy, clinical charting methods, and clinical best practices.
Demonstrated ability to lead and work collaboratively on an interdisciplinary team.
Experience with Department of Mental Health and other government funding agencies.
Excellent verbal and written communication skills.
Clean driving record.
Excellent data management skills.
