Rapid Rehousing Case Manager – Jersey City – York Street Project

York Street Project is a community non-profit organization with a mission to take on the highly visible factors of Education, Housing, Job Readiness and Childcare that block women from creating self-fulfilled, self-sufficient lives. At the same time, we also take on the less visible factors, healing the hidden damages that the cycle of homelessness perpetuates. Every year our programs give nearly 300 women and children the tools they need to begin their journey toward a life of self-sufficiency. Currently we have one (1) role open for a full time salaried Rapid Rehousing Case Manager. The Rapid Rehousing Case Manager provides housing and case management services in order to stabilize homeless individuals. Identifying barriers to long-term housing and obtaining the resources necessary to address these barriers. Providing supportive services to help a household secure and maintain long-term housing

Days and Shift

Monday – Friday during standard business hours – some off shift work as needed

Requirements
ü Bachelor’s degree in human services (i.e.: social work or psychology) Masters Preferred
ü Must be dependable and reliable, detail oriented, and provide effective follow through
ü Excellent organizational, communication, and interpersonal skills
ü Work well under pressure and have the ability to deescalate, mediate and constructively diffuse tense situations effectively
ü Proficiency with Microsoft Office Suites
ü Bi-Lingual English/Spanish
ü Valid driver’s license and personal vehicle
ü 2 years case management experience or working in social services setting

Key Responsibilities
Work with Rapid Rehousing Residents
conduct home visits
liaise with local Housing Authority, create and maintain relationship with local landlords
interview and assist residents with benefits, entitlements, basic needs and be a referral resource and assist resident with Job Readiness and employment
Resident Advocacy
liaise with local, state and federal agencies for issues impacting Residents
mediate with Housing Authority, Welfare and other social service agencies
Administrative Responsibilities
Input date into management information system, prepare and run routine reports, regular correspondence with stakeholder agencies, service and discharge planning
Perform other duties as required
