Project Associate – Partners in Care Foundation – San Fernando, CA

Job Summary Under the general direction of the Program Director performs a wide range of professional duties to support and expand Evidence-based Programs. Duties include building relationships with current and new partners to expand program accessibility to older adults, leading evidence-based workshops in the community, scheduling workshops, recruiting lay leaders and participants, ensuring timely submission of evaluation materials and maintaining lay leader registry.

Provides a full range of clinical services to include assessment of individual and/or departmental needs; short term treatment, referral, crisis intervention and psycho-educational training.

Contributes to design of program content and policies addressing family, personal and work related issues of family caregivers. Develops and conducts training/education sessions for family caregivers.

This is a 100% FTE position.

Responsibilities:

1. Participates in strategic planning, presentations and marketing efforts to introduce and embed evidence-based programs into prospective organizations.

2. Liaisons with licensed sites in California to link organizations and leaders with one another to share resources and common goals.

3. Provides input to Program Director to prepare “hot news” for monthly conference calls and progress reports according to grant timelines.

4. Works with evidence-based program lay leaders and project sites to schedule workshops, manage lay leader registry, and recruit sites and participants.

5. Provides research support, fact checking and content editing for reports, proposals, letters of intent, letters of support, MOUs and other written content.

6. Offers in-person and phone call support to lay leaders and organization coordinators to ensure fidelity of the model, and participation and satisfaction with the program.

7. Responsible for co-leading workshops as needed.

8. Conducts caregiver assessment and diagnostic sessions for individuals, couples and families. Provides referral, short term counseling, case management and crisis intervention services as appropriate. Submits reports to program as mandated by law. Leads clients to development of strategies for conflict resolution.

9. Reviews and evaluates client progress and establishes follow-up contact

10. Maintains confidential client records.

11. Manages the delivery of services to targeted program participants or beneficiaries.

12. Maintains client records and documentation of services.

13. Plans and conduct caregiver support groups, education/training, and outreach. (in-person, phone, internet)

14. Support and work with volunteers and interns.

15. Other duties and responsibilities as assigned.

Minimum Qualifications:

1. Master’s degree from an accredited university in applicable fields (social work, gerontology, public health, etc).

2. Background and experience equal to at least two years in community development, gerontology, and/or other professional disciplines related to health and human services. This experience must have included responsibility for coordinating programs/projects and development of new project initiatives.

3. Highly motivated with minimal supervision and enthusiastic about working in a team environment to improve the lives of older persons.

4. Strong presentation skills to promote/market evidence-based programs to prospective organizations

5. Prior participation in collaborative groups and projects.

6. Strong follow up skills

7. Strong computer skills including Excel and Power Point.

8. Bilingual in Spanish a significant plus.
