POSITION: Program Coordinator II

DEPARTMENT: School Based Health Center (SBHC)

STATUS: Non-Union, Non-Exempt

POSITION SUMMARY

The Program Coordinator II (PCII) will be a valuable member of the NAHC SBHC team by providing coordination, project planning and administrative support for the SBHC Department. The SBHC Department partners with three school districts to provide comprehensive health services at nine SBHCs. The SBHCs reduce health inequities by focusing on prevention and youth empowerment. NAHC is seeking a bright, motivated, detail-oriented individual to work closely with the SBHC Department Administrators and the SBHC Team to support our vision.

DUTIES AND RESPONSIBILITIES

Member services

Greet all patients in a courteous and professional manner to create and maintain a welcoming atmosphere.
Schedule patient appointments appropriately, answer and return telephone calls in a timely manner, and inform patients of processes and procedures.
Register patients into the practice management system (PMS) in a manner that ensures accuracy and thoroughness and update patient registration information once per year at minimum, or as information changes. Inform patients of Notice of Privacy Practices and obtain the patients signed acknowledgement statement.
Screen patients’ eligibility for possible care coverage (e.g., county or state programs; sliding scale) and facilitate enrollment into appropriate publicly-funded programs and follow enrollment and program guidelines.
Responsible for entering accurate assigned payer codes in (PMS).
Confirm patients’ insurance eligibility on all scheduled appointments prior to scheduled appointments.
Check-in patients for their scheduled appointments in a timely manner and complete all flows for the patient appointment such as; verifying patient demographics, processing payments, insurance verifications, payer code selection, printing encounter labels, and pulling charts as appropriate.
Maintain EHR files by creating and editing events, EPM appointment book.

Maintain Electronic Health Records Files

Manage provider schedules by applying provider scheduling templates in departmental Electronic Practice Management (EPM) system, NextGen, appointment scheduler book.
Create/edit events, categories, resources, and templates for department EPM appointment scheduler book and update staff multiviews in EPM appointment book.
Ensure that all required data strategies, collection, and documentation of program objectives and outcomes occur in a timely manner across sites.

Program Coordination

Support the coordination of the day to day functioning of the 9 SBHCs,
Engage regularly with the Program Manager(s) and Program Coordinators to ensure the continuity of processes a services and alignment of service in compliance with department, agency, and funding agency regulations.
Support the promotion of community events, groups, and other activities with school administration and partner agencies.
Assist Program Coordinators and PM in their collaborative work with school staff and school administrators to assess student needs and facilitate referrals.
Coordinate the department’s school screening efforts (dental, vision, hearing) in collaboration with partner agencies and schools.
Support SBHC department as needed with miscellaneous administrative duties such as data entry, tracking information, and scheduling activities.
Actively participate in staff meetings, workshops, and trainings; as well as coordinate activities with collaborating agencies.
Support Program Manager in coordination of community meetings, workshops and trainings as required for the conduct of grant projects and ensure that these meetings operate in a timely and effective manner.
Assist with training and implementation of clinical and administrative policies and procedures so that application are consistent across sites.
Work with site managers to coordinate external audits and regulatory requirements such as VFC, CHDP, CLIA waiver and immunization audits.
In collaboration with the Credentialing Associate and Licensing and Compliance Officer work to ensure compliance with all regulating and licensing bodies.
Represent the SBHC Department in meetings, task forces, and partner collaborations.
Meet regularly with the Department Director and Associate Director to ensure services are aligned with vision of the agency and in compliance with department, agency, and funding agency regulations.
Work with the NAHC Grants and Contracts Administrator to ensure grant and contract compliance.
Manage administrative functions for program staff regarding salary increases, FTE changes, salary allocations using the correct protocol and forms.
Manage all administrative functions connected to school based operations such as purchase requisitions, check requisitions, purchase orders, etc. for any monies being spent out of the school-based grants or third party funds.
Support the Department Director and Associate Director by performing administrative duties related to program planning, meeting coordination, supplies and equipment ordering.
Responsible for ensuring Emergency Disaster Response and Safety Programs are implemented and sustained.
Participate in and effectively works with Safety Committee to promote, teach, develop and maintain a continually improving safety culture (IIPP – Illness & Injury Prevention Program).
Ensure site implements and documents mandatory annual safety trainings and drills. Conduct safety trainings at New Hire Orientations and across the agency as directed by the Safety Committee.
Conduct walk-through of clinic safety items at sites and orient new staff on site safety and emergency preparedness protocols.
Member Care: Demonstrate understanding and apply working knowledge of safety policies and ensuring safe member practices.
Employee Safety: Safely performs all duties; follows required protective protocols to ensure personal safety as well the safety of others.
Must maintain compliance with ergonomic safety standards; be mindful of posture and regularly practice ergonomic stretches.
Quality Improvement:Actively participate in internal quality improvement teams and work with members proactively to drive quality improvement initiatives in accordance with the mission and strategic goals of the organization, federal and state laws and regulations, and accreditation standards, when assigned.
Work well under pressure, meet multiple and often competing deadlines.
At all times demonstrate cooperative behavior with supervisors, subordinates, colleagues, clients and the community.
Other duties as assigned by Supervisor.
Bachelor’s degree in public health, social work, health education, biological sciences or a related human service field or equivalent work experience in a community-based program.
One to two years (1-2) experience with program development, administration, coordination and grant implementation.
Excellent organizational skills and attention to detail.
Able to implement programming and activities as directed and required to support grant requirements.
Fluency in both English and Spanish, both written and verbal, is required, per operational need.
Flexible and able to problem solve and work with minimal supervision.
Ability to work independently and as part of a multi-disciplinary team.
Able to work effectively with youth, school district personnel and staff to coordinate health center services.
Ability to work collaboratively with a broad range of constituents.
Excellent social, verbal, writing and interpersonal skills.
Experience with Windows 2000, Microsoft Office, Excel and Word.
Experience working with multicultural population.
Sense of humor a plus!
Must possess a valid CA driver license and reliable transportation.
