Dual Diagnosis Case Manager- OPCC – Santa Monica, CA

Under the supervision of the Program Manager, the Dual Diagnosis Case Manager is responsible for:

Responsibility:

Provide case management to clients that have dual or multiple diagnoses.
Refer target clientele to appropriate services, per guidelines of the Dual Diagnosis Program.
Develop effective, trusting relationships with clients that facilitate provision of case management services.
Provide case management and aftercare/follow-up services to clients placed in permanent housing through Dual Diagnosis and Shelter Plus Care programs.
Provide information, referral and social service advocacy as needed.
Maintain accurate records and statistical data (manually and via HMIS).
Provide information, referral and social service advocacy as needed.
Collaborate with Dual Diagnosis Program team members and outside agencies as necessary.
Participate in all case file, staff meetings, trainings and retreats.
Participate in on-going self-evaluation, program evaluation, development and implementation.
Other Duties as assigned.

Qualifications:

BA or equivalent work experience.
Demonstrated knowledge of and experience in dealing with issues faced by the population served (homelessness, mental illness, substance abuse, medical issues, and domestic violence).
Experienced in group facilitation.
Knowledge of social service agencies, especially on the Westside, including government benefits/Welfare system.
Skilled in non-violent crisis intervention.
Computer literate – able to use HMIS system for client file management.
Ability to self-motivate and work as part of a team.
Bi-lingual English-Spanish preferred.
Valid California Driver’s License required.
Great organizational skills a must.
