Hours: Monday – Friday 9:00AM – 5:00PM; Some evenings, weekends and holidays may be required.

Home Now Intake Specialist

Summary: This position serves the Home Now Program by managing the enrollment process of all new referrals of individuals experiencing homelessness in DC that are interested in receiving services through the Home Now Program. Home Now is a rapid re-housing program that provides short-term case management and financial assistance to individuals in DC experiencing homelessness to obtain permanent housing quickly. This position assesses individuals for eligibility based on program guidelines and criteria. The Intake Specialist is responsible for identifying new participants for program enrollment via the DC Coordinated Entry system. The Intake Specialist will conduct screen-in surveys with each potential individual and will submit all eligible screen-ins to the Project Coordinator for caseload assignment to case managers. The Intake Specialist will provide referrals to other community resources to individuals who do not meet the eligibility requirements of the program. The Intake Specialist will complete VI-SPDAT assessments and enter them into HMIS as needed. This position will include building and maintaining relationships with local agencies in the community. In addition, the Intake Specialist will be responsible for conducting outreach to local shelters and organizations to provide information about the Home Now program and education on the rapid re-housing model.

Qualifications:

·Bachelor’s degree and at least 1-year experience in a similar setting.

·Understanding and/or experience working with the homeless population.

·Experience with outreach to local and government agencies.

·Experience with HMIS preferred.

·Ability to assess needs.

·Ability to develop resources.

·Excellent written and verbal skills.

·Ability to work in a culturally diverse environment.

·Ability to work well under pressure.

·Ability to work as a team player.

·Valid driver’s license and vehicle are required

Responsibilities:

Assessment of all referrals made to Home Now Program for eligibility of the program.
Complete screen-in surveys via telephone and in the field, as needed.
Maintain participant program enrollment data on Google Docs and HMIS.
Engage participants and assess individual needs, preferences and strengths.
Referral of services to meet needs of participants who are not eligible for services by providing a warm “hand-off” to community providers that can meet their needs.
Develop and maintain relationships with local social services and public benefit agencies, shelters, faith-based and community-based organizations serving individuals who experience homelessness.
Market services provided by the Home Now program.
Conduct outreach and community-based informational sessions on the Home Now program.
Participate in DC Coordinated Entry team meetings.
Participate in multidisciplinary meetings and regular supervision.
Demonstrate empathy and flexibility in responding to participants’ needs and requests.
Attend professional trainings and conferences as needed.
Perform all other duties as assigned.

Specific Requirements for this position:

Employee must have his/her own transportation to carry out job duties and responsibilities. The vehicle must meet legal requirements (insured, registered, etc.) and the employee must have a valid driver’s license.
