Success Coach / Case Manager – Interfaith Housing Coalition – Dallas, TX

Job Description

To provide guidance, support, and accountability using coaching methodology to the program residents at Interfaith in order to ensure their compliance with program guidelines and most importantly, their ultimate success in the program and life. Key responsibilities include helping resident to set and achieve goals, conducting weekly coaching sessions to help residents stay on track, coaching to help residents to identify solutions for challenges, and providing accountability to correct behavior that is counterproductive. The ideal candidate will possess a Bachelor’s Degree or higher in social work, psychology, or a related field, desire to work in a challenging yet fulfilling environment, and possess the discipline necessary to accurately complete the reporting and documentation associated with the position.

Reports To: Director of Family Services, Home & Hope

Responsibilities:

Assists with development and follow-up of goals with each family in the program
Guides families through appropriate tracks and benchmarks completion
Conduct weekly coaching sessions with families and maintain coaching notes relative to family outcomes.
Connects and assists residents in locating appropriate community resources that are needed to aid them in achieving their goals and makes proper referrals for service
Serves as liaison for coordination of services with internal providers that assist with resident programming services
Participates in weekly meetings with Director of Family services and case management team
Serves as the primary contact and advocate for the families during their programming
Monitor program participation in career services, financial coaching, life skills classes, and child care services.
Ensure files are in order/accurate and HUD compliant
Abides by agency’s values, ethics and confidentiality agreements
Update and submit weekly progress report on families

Requirements:

Bachelor’s Degree in Social Work or related field. Must have at least two years of previous case management experience. LSW or Master’s Degree, a plus.
Must be proficient in use of Microsoft Office Programs
Must have strong verbal and written communication skills, strong listening skills and ability to exercise empathy and strong boundaries
Some knowledge and experience with mental illness, issues regarding homelessness, and addictions is extremely helpful and preferred
Experience serving diverse populations
Must be flexible with work schedule

Salary Range:
$30,000 to $40,000 annually

Required experience:
Case Management: 2 years
