Lead Case Manager – New Directions for Veterans – Los Angeles, CA

Summary: The Case Manager/Outreach worker works directly with veterans and their families by helping to outreach to the community and developing goal plans to meet both the veterans’ immediate needs housing needs, providing crisis intervention and resource referrals. Maintains computerized case management and hard copy files and is responsible for special projects as assigned.

Job Duties:

Assess new clients during screening, intake and orientation
Assess program qualification criteria of client and aid in qualifying or referring to outside resources
Develop individualized goal plans and monitors progress in achieving goals
Assist veterans in goal planning of housing attainment and sustainability including job training, education, and money management income and benefits attainment.
Provide ongoing support, crisis intervention and housing stability support and resource referrals.
Coordinate housing, medical, legal, dental and/or psychological services as/if needed
Work consultatively with other staff to ensure appropriate levels of treatment/support
Keep client files current through the computerized care management system and maintain ongoing files both on computer and hard copies
Conduct home visits to support client in housing stabilization.
Review check requests from other SSVF Cs Mgrs. and submit to supervisor for final approval.
Monitor data quality of the other SSVF Cs Mgrs.
Produce Monthly Participant data reports, troubleshoot and amend data errors when needed.
Provide supportive services to other Cs Mgrs. when needed. Demonstrate departmental and company policy consistently and professionally.
Participate in street outreach to identify homeless veterans and present the program as an alternative to homelessness.
Liaison with the VA outreach departments, community outreach departments, VASH department and the VA benefits and hospital departments when applicable.

Participate in conferences, workshops, special projects, staff meetings, and other duties as assigned

Job Requirements:

Knowledgeable of substance abuse issues, therapeutic interventions, and treatment planning, particularly as relevant to Therapeutic Communities
Some knowledge of affordable housing programs such Section 8 housing, VASH voucher and Shelter Plus Care programs.
Knowledge of the 12 steps of Alcoholic Anonymous or other fellowships
Working knowledge of community resources as they relate to homelessness and veterans
Experience and knowledge in working with a multidisciplinary treatment team
Word processing skills required
Veterans preferred
Three (3) years of work experience is preferred
Minimum of one (1) year clean and sober, if appropriate
Educational background can be diverse; however, a Bachelor’s Degree is preferred.
