About the Agency: Health Imperatives is a non-profit community-based agency, predominantly serving families and individuals in Southeastern, MA. Our mission is to improve the health and wellbeing of low-income or vulnerable families and individuals in Southeastern Massachusetts. We accomplish this mission through administering both health and violence intervention and prevention programs and services.

TITLE: WIC Community Coordinator

HOURS: Full-time, flexible, some evenings & Saturdays (35 hours/week)

PROGRAM: Plymouth WIC

SALARY: $16.00 per hour

BENEFITS: Medical, dental, holiday, paid earned leave, retirement, etc.

DUTIES: Under the direct supervision of the Program Director, the Community Coordinator’s responsibilities will include the following:

Develops and maintains the MA WIC Community Network Database

Develop and maintain community liaisons, as well as a comprehensive listing, with health and human services providers and advocacy organizations in community served by the local WIC program to facilitate & carry out referrals between these groups & WIC;

Build an active, coordinated referral network & collaborative, working relationship between community agencies & groups & WIC

Develop monthly and annual outreach plans and document monthly activities

Develop and utilize outreach tracking tool that measures new participants linkages to program;

Evaluate outreach effectiveness regarding client directly linked to PLY WIC as a result of outreach

Facilitate or conduct periodic in-services for staff and/or clients at local welfare offices, health & human services providers & other community agencies that deal with the WIC population and/or nutrition & food, i.e., Head Start, nursery or day care, grade schools, churches, EFNEP, neighborhood associations, Housing Authority, etc., as well as local pediatricians, obstetricians, & other health care providers;

Provide WIC program, education & outreach materials to local agencies & advocacy groups;

In coordination with the Program Director, represent WIC on committee & sub-committees of local health and human services coalitions;

Participate in community health fairs, workshops & other public forums;

Attend PLY WIC staff meetings;

Attends Community Coordinator quarterly meetings, trainings, and in-services; and

Other duties as assigned by the Program Director.

REQUIRED QUALIFICATIONS:Associates Degree in Human Services, Community and Public Services, Community Organizing, Marketing for Health and Human Service or Health Promotion .Knowledge of community and health and human services in area. Sensitivity to the needs of participant population. Good oral and written English language skills. Strong presentation skills.

PREFERRED QUALIFICATIONS: 2 Years of experience working in the health or human service field in an outreach and collaboration or marketing role. Bilingual Spanish/Portuguese desired.

Strong Knowledge of Greater Plymouth Health and Social Services
