Medicaid Service Coordinator – Harris Hands Community Services Inc – Bronx, NY

Job Description:

Part Time Medicaid Service Coordinator needed IMMEDIATELY (to work for an expanding Bronx based agency to provide Medicaid Service Coordination to people diagnosed with intellectual disabilities in Bronx, Manhattan, Queens and Brooklyn.

Must be a self starter, compassionate, able to work independently, able to multi task, meet deadlines and have a pleasant personality. The ideal candidate is looking to grow with a start up agency and help assist in the expansion of the Medicaid Service Coordination Department as well as future up and coming departments.

Duties include:

Providing MSC/Case Management to people diagnosed with intellectual disabilities.
Facilitating ISP meetings semi annually.
Conducting at least quarterly face to face visits and monthly correspondences.
Meeting OPWDD compliance.
Managing and maintaining Medicaid benefits.
Seeking services appropriate to individual’s needs.
Representing the agency in a professional manner at all times.
OPWDD mandated MSC trainings and certifications.
Intakes and outreach of new clients for Brooklyn, Bronx, Queens and Manhattan.
Fundraising, and various trainings and meetings.
Perform additional duties as assigned.

Skills:

Candidate must have at least 1 year experience as a Medicaid Service Coordinator/Case Manager and/or 1 years experience working with people diagnosed with developmental disabilities. Bachelor’ degree in a HEALTH OR HUMAN SERVICES field MANDATORY. Applicant must be able to communicate in Spanish fluently.

Other Comments:

The part time position will require someone who can speak and comprehend spanish fluently. The person must be available during traditonal working hours and must have flexible availabilty. The part time position is 20-25 hours per week.

Must have own transportation and be able to travel to various locations within the tri-state area. Candidate must be available during normal and non-traditional business hours for meetings and needs of the agency and clients. Person must pass mandatory background checks.
