Job description
Carolina Medical Home Network-ACO (CMHN-ACO) is a Medicare Shared Savings Program (MSSP) in the 2015 cohort. CMHN-ACO membership includes six (6) North Carolina federally qualified health centers and the primary care association for NC. CMHN-ACO is seeking an individual or firm to manage day-to-day ACO operations, under an initial contract ending 12/31/2017. As this is a contract position, it does not include a benefits package.

Key responsibilities will include building a project management infrastructure for efficient information sharing and deliverables tracking; defining and managing project work plans; identifying and coordinating the work of team members; developing key milestones and timelines; identifying and managing project issues, risks and decisions; developing workflow diagrams and procedures; utilizing change management strategies for project and process implementation; developing progress reports and presentations and other methods to communicate project progression; creating dashboards, facilitating committees/workgroups, and ensuring timely project deliverables and outcomes.

The ACO Program Manager will report to the CMHN Executive Director. The Program Manager will act as the single point of accountability for delivering large scale, complex, and cross-organizational projects and/or program strategy deliverables on time and on budget simultaneously for the ACO. The Program Manager is expected to direct the daily activities of internal and external team members to design and develop approaches to projects and/or program strategies that they implement themselves or are implemented by others, ensuring that the strategic direction are in line with business objectives.
