Supervisor – NYFSC Case Management Program

DESCRIPTION
New York Foundation for Senior Citizens, Inc. is dedicated to helping New York’s seniors enjoy healthier, safer, more productive and dignified lives in their own homes and communities and to help them avoid premature institutionalization.

The Case Management Program, located in lower Manhattan, serves senior citizens 60 and over who reside in Manhattan Community District’s #1, 2, 3, 5 & 6.

The Supervisors responsibilities include:
Ensuring all client case records contain all information, case notes, referrals and assessment reports in accordance with the NYC Department For The Aging standards
Weekly one-to-one supervision with assigned Case Managers
Perform all tasks requested by the Program Director to support the team functioning of the Case Management Program

Skills:
MSW required, LMSW/LCSW preferred. Understanding and knowledge of benefits/entitlements and previous supervisory experience preferred. Excellent written/verbal communication, computer, and organizational skills also required. Bilingual preferred (English/Spanish, English/Cantonese, English/Mandarin, English/Polish, English/Russian), however all are encouraged to apply.
