Seeking a Swing Shift Clinical Care Coordinator – San Francisco Child Abuse Prevention Center

The mission of the San Francisco Child Abuse Prevention Center (Prevention Center) is to prevent child abuse and reduce its devastating impact. The Prevention Center implements a three-prong strategy to end child abuse: (1) providing care directly to children and families through high-quality Children & Family Services; (2) educating the community—children, caregivers, child-serving professionals, and the public—through proven Community Education programs; and (3) coordinating Strategic Partnerships that promote citywide systems improvements and enhance our capacity to protect children, including creating a Children’s Advocacy Center.

The organization has an annual operating budget of approximately $6 million and is supported by approximately 60 staff, interns, and consultants and more than 80 volunteers. The Prevention Center owns its building, an historic firehouse in the Haight-Ashbury neighborhood of San Francisco, near Golden Gate Park. The organization has a second site on Third Street in San Francisco’s Bayview neighborhood.

OVERALL SUMMARY
This position is responsible for leading and coordinating the efforts of a multi-disciplinary team of service providers to build families’protective factors. It is the key position in our Integrated Family Services model. This includes facilitating regular team meetings; assessing family risks, strengths, and challenges; as well as developing service plans and providing counseling and care management services. This position also staffs Supportive Family Services including drop-in hours and family events as well as administrative support to comply with government contracts and private grants.

ESSENTIAL FUNCTIONS

Integrated Family Services:

Serves as the primary counselor to families enrolled in Integrated Family Services to build families’ protective factors.

Holds primary responsibility for: 1. assessing families’ functioning, 2. monitoring progress toward outcomes, and 3. graduating families from Integrated Family Services, using tools defined in the Integrated Family Services model. Collaborates with other Clinical Care Coordinators to triage families eligible for Integrated Family Services and enroll them based on need and other relevant factors.

Leads and facilitates a weekly Service Delivery Team meeting composed of staff from all program areas within the Children & Family Services (CFS) Department, using a format common to all Service Delivery Teams.

Develops service plans in collaboration with clients and Service Delivery Team, according to Integrated Family Services guidelines.

Provides intensive care management (including home visiting) to clients with multiple challenges, including families referred by Child Protective Services.

May provide short and long-term counseling for adult and/or child clients, depending on care coordination caseload.

Maintains an on-going care coordination roster of 10-15 clients.

Supportive Family Services

Serves as Coordinator of the Day for the Parent Drop-In program one day per week. Provides oversight and coordination of the Parent Drop-In program, including supporting other staff and interns providing drop-in services.

Coordinates family support center services including Food Pantry, Emergency Needs Support, information and referrals.

Meets with clients not enrolled in Integrated Family Services on a drop-in basis while daytime Parent Drop-In is open, including performing intakes and making referrals.

Assists and supports Parent Drop-In program as needed including helping set up and break down.

Keeps records of all services provided in compliance with legal, agency, and funding requirements.

Participates in outreach activities as well as provider and community meetings to increase referrals and community awareness of the program.

Attends all Care Coordination team meetings as well as other staff meetings and trainings as directed by supervisor

MINIMUM REQUIREMENTS & QUALIFICATIONS

At least 3 years hands-on experience providing direct services to youth (0-12), parent(s) and families is required

Licensed Clinical Social Worker, LMFT, or a Master’s Degree in Psychology, Counseling, or Social Work; or equivalent job related experience required

Knowledge of the effects of child abuse, child development, effective parenting practices, and family dynamics required.

Knowledge of community resources in San Francisco, cultural competency practices, and current client outcome measurement practices required.

Previous experience working with youth and their families that are underserved, marginalized and high risk with trauma histories strongly preferred.

Proficient in Spanish and/or Cantonese strongly preferred

Ability to build effective working relationships with staff, set clear expectations, and provide both positive and negative feedback in a timely manner.

Ability to effectively collaborate with clients to complete assessments and develop appropriate service plans that support their protective factors.

Ability to use both qualitative and quantitative data to inform service planning and monitor client progress towards achieving outcomes/goals.

Ability to work with diverse staff and client populations.

Ability to prioritize work effectively in an environment with multiple competing priorities.

Ability to work tactfully and effectively with representatives of city agencies, community-based organizations, and the community at large.

Ability to operate within and lead a complex service delivery structure.

Ability to handle crisis situations.

Ability to use excellent time management and organizational skills.

Ability to use computer skills: word processing, using digital recordkeeping systems to collect and track data, and internet search capabilities.

Demonstrated experience working with diverse cultural backgrounds.

Friday 9:00am to 5:00 pm
