Registerd Nurse Medical Coordinator – National Health Foundation – Los Angeles, CA

National Health Foundation (NHF) is an independent, tax-exempt 501(c)(3) public charity dedicated to improving and enhancing the healthcare of the underserved by developing and supporting innovative programs. NHF’s Pathway Recuperative Care, located in Los Angeles County provides hospitals a safe discharge option for homeless patients who are not sick enough to remain in the hospital, but too sick for a shelter. Pathway Recuperative Care operates at two locations, Mid-City and La Puente. This position will report to the Los Angeles Site (most of the time). The R.N. Medical Coordinator, as part of an interdisciplinary team, will provide strong nursing and assessment skills to our recuperative care clients.

ESSENTIAL DUTIES & RESPONSIBILITIES

Review all patient referrals for recuperative care admission and ensure approved patients meet the admission criteria

Ensure daily rounds are conducted to review care plans, organize transportation to follow up medical appointments; monitor client care services

Review/update client care plans

Coordinate follow up medical visits

Maintain follow up monitoring and tracking of post recuperative care clients

Ensure crisis situations are handled and the appropriate paperwork completed

Orders medical, office and client welfare needs

Complete and maintain accurate and up to date charts in electronic databases and client charts

Close out all monthly expenses and submit reports in a timely manner

Cooperatively work with other members of the team including active participation in department and agency staff meetings and trainings

WORK EXPERIENCE REQUIREMENTS

Must have three (3) years solid nursing experience in a healthcare setting

One (1) year working with underserved populations

Must be able to work autonomously and as part of a team in non-traditional setting, have a thorough knowledge of community resources, and be a strong patient advocate

Proficient in computer applications and electronic records, MS Word, Excel, Outlook

QUALIFICATIONS

Must possess a nursing degree from an accredited college or university

Must possess a current, unencumbered, active license to practice as an RN in California

Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations and guidelines that pertain to nursing care facilities

Maintain confidentiality of all resident care information in accordance with HIPAA guidelines

Should be flexible and culturally sensitive (to race, ethnicity, religion, gender, and sexual orientation, as well as to the culture of poverty and of homelessness)

Excellent communication and organizational skills
