Re-Entry Case Manager (P/T)
Newark Emergency Services for Families, Inc. (NESF) is a private not-for-profit organization located in Newark, NJ that offers an array of different services through its integrated services delivery system.
NESF is seeking a Part-Time Re-Entry Case Manager. The Re-Entry Case Manager is responsible for providing case management and referral services to consumers re-entering society from incarceration.

Specific responsibilities include:
o Conducing comprehensive assessment of all consumers
o Ensuring that cases are handled within the timeframe and manner prescribed by the funder
o Developing individual Service Plans for each client
o Orienting consumer to program and service and requirements for services available through Reintegration program
o Providing information and referrals in relation to the Re-entry population
o Preparing and maintain consumer files and internal client database system
o Processing internal and external consumer referrals
o Assisting with consumer outreach and community education
o Attending staff meetings and trainings as required
o Developing and maintaining linkages to collaborative agencies to assist consumers with rental agencies, shelter providers, employment agencies, health care providers, education/trade programs and County welfare
o Providing Programmatic reports on monthly basis
o Conducting consumer follow-up to ensure successful transition back into society
o Bachelor’s Degree in a Human Services Sociology, Psychology or related field
o Good Written and Verbal Communication Skills
o Experience working in an environment with high consumer volume
o Ability to build a rapport with individuals/families
o Ability to utilize support systems and services agencies to make referrals accordingly
