Organization: Street Level Health Project is an Oakland-based non-profit organization dedicated to improving the health and well-being of underserved, urban, immigrant communities in the Bay Area. Street Level is “a safety net of the safety net” and serves as a point of entry to accessing health and other services for the uninsured, underinsured, and recently arrived, as well as an advocacy space for low-wage immigrant workers. We believe health and well-being go beyond medical care, and in addition to medical, mental health and herbal/nutrition services, our center provides access to referral/resource navigation and support, healthy food, social connection, and opportunities for community engagement and advocacy.

Health Access Program: The Health Access Program (HAP) focuses on healthcare and health advocacy, and is a collaborative effort of a diverse team including a medical director, volunteer healthworkers, mental health counselors, a nutritionist/herbalist, interpreters, volunteer medical providers, students in preceptorship and external partners.

Co-Manager Position: The Health Access Program Co-Manager will work closely with the existing HAP Co-Manager to facilitate the day-to-day operations of the clinic, advocate on an individual and systems level for community members and support the Health Access team. Recognizing the complex demands and steep learning curve of this position, the first 6 months will focus on gaining experience in clinic coordination, individual level health systems advocacy, and an overall understanding of Street Level’s organizational model. Upon completion of 6 months, the position will transition into a full Co-Manager role with increasing responsibility and involvement in program development, systems-level advocacy and community organizing.

Primary Responsibilities:

Clinic Coordination (one day per week, 25-30% of total time)

Supervise clinic flow, quality of care, and provide problem-solving support to the clinic team, including volunteer healthworkers, interpreters and providers
Provide training, support, feedback and leadership development for volunteer healthworkers
Connect clients with appropriate referrals for healthcare and social service resources, and provide in-clinic case management and referral follow-up
Offer client education and skills building for navigating healthcare system, accessing resources, identifying potential barriers and tools to overcome them
Fill in as needed for healthworker roles, such as patient intakes, clinical skills and data entry
Cover clinics for the other Health Access Co-Manager as needed

The following responsibilities are shared fully with a 32/hour per week co-manager:

Health Access Program Administration and Development (20-30%)

Support, collaborate, plan and share information with HAP Co-Manager
Facilitate day-to-day administrative needs of clinic, such as responding to medical records requests, ordering supplies, assessing volunteer needs and managing the space
Collaborate with HAP Co-Manager, Medical Director and other SLHP staff to help vision, develop and implement medical provider transition plan
Collaborate with the HAP Co-Manager and administrative team to ensure funding opportunities are aligned with programmatic priorities
Provide problem-solving support, guidance and feedback to Health Access staff including the nutritionist/herbalist, mental health program coordinator and interpreters
Recruit, coordinate and support volunteer medical providers
Facilitate collaborative relationships with partners such as healthcare schools, clinics and hospitals
Collaborate with Co-Manager on Salesforce database management, data tracking and reporting
Develop and improve our clinical health education materials and practices
Grow and cultivate our healthcare referral network with other organizations
Coordinate and help facilitate monthly HAP meeting

Patient Care – Case Management and Referral Follow-up (20-25%)

In collaboration with Health Access Co-Manager and other staff, provide medical case management, referrals, referral follow-up support and individual-level advocacy to community members
Collaborate with non-clinic program staff/volunteers to ensure continuity of care in accessing services and resources

Advocacy and Leadership Development (10-20%)

Develop and maintain relationships with local organizations and resources; provide feedback and advocacy around access-to-care issues
In collaboration with other staff, support leadership development of community member leaders
Advocate with Street Level’s communities at the level of local, county and state organizations

Organizational Support (10-20%)

Participate in weekly staff meetings. Attend other all-staff and program staff meetings as needed
Help community members seeking information, assistance, referrals, etc. both in person and by phone
Support other SLHP staff / programs as needed with both direct services and feedback/problem-solving
Help with organizational events as needed (e.g. community events, fundraisers, or open houses)
Participate in organizational visioning, planning, decision-making (e.g. strategic planning process)
Occasional street outreach with day laborers in Oakland about SLHP and HAP services

Required Skills and Qualifications:

Must be bilingual in Spanish and English, with strong oral and written communication skills. No exceptions. Bicultural and/or native Spanish speakers strongly encouraged to apply.
Minimum 2 years of experience in clinical, medical or health work (experience more important than formal degree or training, experience does not have to be in an official clinic setting)
Experience working in community with people who are immigrants, undocumented, poor, limited/non English speakers, limited/no literacy or formal schooling, may be dealing with substance use and trauma, and face many barriers to accessing and navigating systems of healthcare and social services
Excellent communication, mediation and group discussion skills
Flexibility, adaptability and creative problem-solving approach
Able to work in a loud, extremely busy, drop-in community center environment
Comfortable being present and supportive to people in crisis
Strong desire and ability to work collaboratively as a team
Ability to handle multiple urgent demands and prioritize them effectively and quickly
High level of independent work, self-direction, initiative and inquisitive learning style
Comfortable working with little direct supervision once initial training period is complete and able to ask for support and feedback when needed
Computer literate and experienced with Microsoft Office

Desired Skills and Qualifications:

Experience in community organizing, advocacy and leadership development preferred
Experience with database management, Salesforce or data analysis
Knowledge of local, community and county resources, social services and health systems
Knowledge of issues specifically affecting day laborers and newly arrived immigrants
Interest in long-term involvement with the organization
Experience/interest in health education, holistic health, nutrition, mental health and/or substance use

Salary, Benefits and Hours:

Position is currently part time at 20 hours/week. Schedule is flexible but priority days will likely be Thursdays (approx.11 hours) and either Tuesdays or Wednesdays (approx. 9 hours).
Compensation will be $15/hour with partial benefits: sick and vacation time accrual based on hours worked, no health coverage.
