Clinic Nurse Manager (RN) – Native American Health Center Inc – San Francisco, CA

POSITION SUMMARY

Under the direction of, and in coordination with, the Lead Physician–and Site Director, the RN ensure the provision of high quality, well-coordinated medical services in a clinical care pod model. The RN will lead a clinical care pod and will perform both supervisory and clinical nursing duties.

DUTIES AND RESPONSIBILITIES

Clinical Duties

Perform clinical nursing duties with high integrity, quality and under the clinical supervision of the Lead Physician, and in coordination with the clinical care pods.

Coordinate member flow for the clinical care pod.

Conduct nursing visits which may include, but not be limited to: wound care, pregnancy tests, injections, BP checks, urine tox screens, pill counts, TB tests, etc. (as directed by providers).Perform basic in-house lab tests; obtain and process specimens for outside laboratories.

Assist and facilitate patients in obtaining medical services, including scheduling patients as appropriate, triaging patients, and assessing patients who walk-in to the clinic seeking services.

Take and record vital signs, recognize any variances, outliers or red flags, and take appropriate action.

Provide patient triage and assessment and obtain health information and relevant data.

Perform EKG, audiometric, Snellen and other necessary patient screening tests.

According to departmental protocol, provide medications and conduct medication reconciliation.

Administer IM, ID, SC, PO, oxygen and nebulized medications as ordered by the clinicians.

Fill in for MA’s as needed, which can include and is not limited to preparing patients to be seen by the clinic practitioner -taking patient history, determination of the chief complaint, and providing patient education.

In coordination with Primary Care Provider (PCP), counsel patients and families regarding health education needs, preventative health and chronic conditions.

Provide case management to NAHC members including but not limited to those with chronic health conditions such as HIV, HEPC and diabetes. Case management duties include, but are not limited to, executing the medical orders of the physician, monitoring labs, calling in prescription orders and coordinating any necessary prior authorizations and insurance issues, educating patients regarding medications and advocating for treatment adherence, and diligently documenting this work in the medical chart. Work with Circle of Healing staff and provide updates regarding member medical care as needed.

Support the providers by processing medication refills according to the medical refill protocol.

Clinic Administration

In coordination with the RN Clinic Manager in Oakland, plan and implement clinic policies, procedures and workflows.

Prepare department for audits (Managed Care Plans, IHS/IHP) as required by auditor tool. Escorts auditor and assist with audit as needed.

Facilitates regular meetings with staff. Involves staff in decision making as appropriate.

Ensure HIPPA compliance in maintenance and storage of medical records.

Work in conjunction with the Site Director, Lead Physician, CMO and RN Clinic Manager in Oakland to oversee the Quality Assurance program to assure that a high standard of medical care is provided and that non-medical procedures and operations are of a high quality and responsive to changing patient needs and populations.

As needed, review and update clinic forms to maintain compliance with relevant local, state, and federal law.

Train all clinic staff in new (and old) policies and procedures, as needed.

Assists and involves clinic staff in education and awareness efforts concerning cultural diversity.

Oversees all aspects of the management of clinic facilities and equipment including maintenance and ordering of clinic inventory, medications and supplies.

Supervises the immunization project to include planning, training, report writing and immunization chart audits.

The RN will ensure patients are seen on a timely basis, identify and communicate barriers to efficient flow and identify possible solutions.

Maintain pharmacy medication log, refrigerator and room temperatures.

Oversee data entry into various programs (e.g. the California Immunization Registry (CAIR), NextGen, i2i). This includes ensuring data entered is done in a timely, accurate and thorough manner.

Ensure quality control over encounters (i.e., superbills) and coding.

Coordinate monthly, weekly and daily schedules for the providers and MA’s in conjunction with the lead physician.

Supervision and Leadership

Ensure orientation and training of Medical Assistants on standing orders, annual required trainings, and on trainings needed based on work performance.

Member Care: Demonstrate understanding and apply working knowledge of safety policies and ensuring safe patient practices.

Quality Improvement: Actively participate in internal quality improvement teams and work with members proactively to drive quality improvement initiatives in accordance with the mission and strategic goals of the organization, federal and state laws and regulations, and accreditation standards, when assigned.

Resolve staffing issues and facilitate conflict resolution for staff under direct supervision. Hire, discipline, evaluate and terminate staff in coordination with the Human Resources department.

Effectively lead and retain your staff by fostering a team environment and providing opportunities for staff to develop new skills

Must maintain compliance with ergonomic safety standards; be mindful of posture and regularly practice ergonomic stretches.

Safety: Responsible for ensuring that all duties, responsibilities and operations are performed with the utmost regard for the safety and health of all personnel involved, including themselves.

Safety: Take appropriate corrective actions to address matters pertaining to employee health and safety that have been brought to their attention.

Works extremely well under pressure, meet multiple and often competing deadlines.

At all times demonstrate cooperative behavior with supervisors, subordinates, colleagues, clients and the community.

Other duties as assigned by Supervisor.

MINIMUM QUALIFICATIONS

Must have a current R.N. license in the state of California, and the license must be in good standing.

Must have at least one year of work experience supervising at least 3 people. Two years of supervising medical support staff is preferred.

Two years’ of work experience in outpatient nursing, preferably in a clinic setting, is highly preferred.

Must be able to communicate effectively, both written and verbally, with varying audiences.

Must be customer-service focused.

Must be able to maintain composure while under pressure and in stressful situations.

Must be detail oriented, and have the ability to set priorities appropriately in order to meet deadlines.

Must be able to make sound judgments and be able to delegate effectively.

Must be comfortable working on computers – including the following software: Microsoft Word and Excel, as well as electronic health records.

Bilingual in Spanish is preferred.
