Position Title: Before Thirty Youth/Young Adult & Shelter Case Manager

Reports to: Welcome Center Assistant Director and Division Director

Category: Exempt

Hours: M-F 9am – 5pm, Some evening/weekend hours, Participation in the oncall rotation.

Qualifications:

·Ability and willingness to work in a diverse and challenging environment.

·Ability and willingness to work as a team player.

·Flexible and passionate about providing a broad spectrum of services to vulnerable populations.

·Bachelor’s degree, BA/BS in human services field or social work.

·2+ years experience with homeless, mentally ill or substance abuse preferred.

·Proficient in Microsoft Word, Excel, etc.

·Strong written and verbal skills.

·Valid driver’s license and ability to operate a motor vehicle.

Responsibilities:

Managing women’s shelters (The Haven Transitional Shelter, and Metro House Women’s Shelter), facilitating entrances into shelter and exits from shelter.
Completing intake and biopsychosocial assessments and interviews for shelters.
Participate in or facilitate entrance meetings required by shelter staff for entry.
Oversees medication management for the residents in shelter if necessary.
Responsible for the input of data into HMIS as well as providing data for internal data reporting systems.
Develop and implement individual case plans for each consumer on caseload and work with each consumer to accomplish goals such as physical health, money management, developing social and community supports, activities of daily living skills, etc.
Responsible for keeping accurate and timely written case notes.
Connect, collaborate, build, and maintain positive relationships with external agencies that provide services to Friendship Place consumers.
Participate in the planning and development of new projects as they relate to consumer services
Assists with volunteers when necessary.

Engage participants, assessing safety, and activating emergency services as needed.
Provide basic need items and assistance to maximize participants’ wellbeing.
Build trusting relationships and rapport with participants.
Based on observations and knowledge develop personalized approach to the service delivery for each participant.

Actively participate in case meetings, staff meetings, and supervisions.
Actively participate in trainings and other events offered by Friendship Place and other agencies.

For the Before Thirty Program:

Interviews, advises, and guides a diverse population of participants to ascertain clinical needs and assess housing needs; interprets and explains regulations, rules, policies, and procedures to participants; may determine participant eligibility for services; apprises participants of their rights, benefits, responsibilities, and obligations under Friendship Place program participation; ensures that applications and other forms are completed accurately and thoroughly.
Assesses participant’s clinical needs, education, work experience, skills, abilities, qualifications, which may include job interest; assesses participant readiness for job referral, classroom training, on-the-job training, and/or support services; attempts to match participants with available employment, training, or other opportunities/services.
Identifies problems/barriers that hinder a participant’s ability to achieve and maintain self-sufficiency; assists participants in resolving and mitigating barriers by identifying the need for other services.
Analyzes information obtained from assessments, tests, and other sources to develop short- and long-term participant goals
Perform other duties as assigned.

Specific Requirements:

You must have a valid drivers license and have your own transportation to carry out job duties and responsibilities. The vehicle must meet legal requirements (insured, registered, etc.).
