Salud Para La Gente (SPLG) provides high quality, comprehensive and cost-effective healthcare to underserved low-income communities in the Monterey Bay area, including Santa Cruz County and North Monterey County. We began in Watsonville 1978 as a storefront “free clinic,” and with the ever growing need for health services, in 1992 it became a federally qualified community health center. Today we are a primary health care network with 12 clinic sites and continue provide high quality services to patients of all ages.

We are seeking a dynamic, organized and detail-oriented Health Education Supervisor to be responsible for planning, implementing and evaluating the delivery of health promotion and education to patients and community members to support understanding of their health and ability to proactively participate in their care. The Health Education Program Supervisor will be responsible for programs that meet the needs of the populations being served including coordinating the implementation of shared medical appointments and group visits; supervision of health coaches; coordinating the identification and use of health education materials and media across Salud sites; training staff to provide health education and directly providing health education to patients.

Responsibilities include, but are not limited to, the following:

Develops, coordinates, implements, and evaluates health education through a variety of approaches for Salud patients to support understanding of health conditions and ability to proactively participate in care including chronic diseases, behavioral health, pregnancy, child development, oral health etc.

Identifies appropriate and effective curriculum, techniques, materials and media to communicate with patients and community members to support self-management and active participation in improving and maintaining health.

Coordinates shared medical appointment (SMA) and group visit program implementation: ensure that SMA and group visits are delivered in an effective and efficient manner to meet the needs of the population served by Salud including patients with chronic diseases, children and their parents and pregnant women. Facilitate and deliver health education at SMA.

Assists with maintaining data and records as required by funding sources, clinic management, and evaluation needs.

Assists with research and preparation of reports, grant proposals, budgets, evaluation instruments, and other required written communications.

Directly provides or identifies training for Salud staff on delivery of health education.

Delivers health care guidance and support directly to patients in group and individual settings.

Develops and maintains relationships with community partners providing health education services to the population served by Salud to promote coordination and access to appropriate and effective services including with: Second Harvest Food Bank, Pajaro Valley Prevention and Student Assistance, Diabetes Health Education Center, Palo Alto Medical Foundation, Watsonville Community Hospital, Counties of Santa Cruz and Monterey Health Departments, Central California Alliance for Health, YMCA, YWCA, Catholic Charities, etc.

Performs under limited supervision with accountability for specific goals/objectives.

Supervises, trains, and evaluates Health Coaches.

Ensures a productive work environment and achievement of goals through strategies and practices in line with the CARE fundamentals of communication.

Other duties as assigned.

MINIMUM QUALIFICATIONS

Bachelor’s (Master’s degree preferred) degree in public or community health and/or health education or related field required
Three (3) years of experience in health education and a minimum of one (1) year of experience performing case management or an equivalent combination of education and experience which would provide the required knowledge, skills and abilities may be qualifying.
One (1) year supervisory or management experience.
Working knowledge of the principles and practices of health education and case management.
Working knowledge of planning, implementation and evaluation of health education programs.
Experience in ambulatory clinical setting and/or Federally Qualified Health Center strongly preferred
Valid CA Drivers License and current vehicle insurance; must be able to travel between sites.
Bilingual English/Spanish
